By participating in this learning program and assessment tool, the individual acknowledges and agrees that:

1. This learning program only covers Tissue Donor Eligibility issues.
2. This learning program does not address all Tissue Donor Eligibility issues. It is designed to provide an overview of Tissue Donor Eligibility as set forth in the relevant FDA cGTP regulations in effect as of May 25, 2005 (excluding reproductive tissue, living surgical donor bone, and autologous tissue standards) and the 2006 11th Edition AATB standards. This learning program does not address any tissue bank or processor specific criteria or any state requirements that may exist.
3. Tissue Donor Eligibility standards as established by the FDA cGTPs and the current AATB standards are subject to change over time. This learning program does not address any such changes.
4. This learning program is designed and intended to be used exclusively as a tool for evaluating an individual’s general familiarity with Tissue Donor Eligibility.
5. No user of this learning program may rely on any part of this learning program or on their assessment results for any purpose other than for evaluating his or her general Tissue Donor Eligibility knowledge.
